Co-Op or Deployment? Because this issue is more common in Co-Op where there's no command posts or firebases to be teleported to on some layers, so game picks death.

Run once in 800x600 then change resolution and run again?

FH2 runs things from their own launcher, you'll have to ask them for more information, sorry.

Only other thing I can think of is that map contains none of the statics in mods\bf2\objects?

Otherwise I tried importing myself and it imported without helpers when objects were extracted.
