Only other thing I can think of is that map contains none of the statics in mods\bf2\objects?

Otherwise I tried importing myself and it imported without helpers when objects were extracted.

Sure you don't have the 'import helpers' ticked? Otherwise objects needs to be extracted from zips for Max to import them.

Some flag layouts like Jabal and Dragon Fly have been changed for v1.6, haven't heard anything on that yet.

You might as well clone the entire map then replace the carrier, it'll make managing lightmaps easier.

You should open an existing heightmap with correct settings, edit it and then save. At least that's the way I know to edit and save heightmaps successfully.
