Trust - High Performance Optical Gamer Mouse GM-4600

Thumb on the 2 side buttons.
1 finger on the left button.
Middle finger on the scrollwheel.
1 finger on the right button.
And my pinky on the right side.

cheep and does the job

that looks really like the first saitek gaming mouse, in fact I think trust is part of Saitek.

I go with the 2 fingers on top.
As a side note, I just did this picture for a similar topic in another forum, so figured I would post it here.
My button setup using uberoptions.

You can't actually hold mine differently. Thumb on the left (Unless you're a lefty ) Three fingers on top and the last one left. Done

It's a MX518 btw..

EDIT:

Now you're asking, I actually don't know how I hold it

I have a standard microsoft optical mouse. I have bound the pressing down of center scroll wheel to activate voice comms (venntilo/TS/etc) so my finger is right there on the trigger if need be. So I have my trigger finger on the left mouse button. Middle finger on the right. Pinky and Ring off to the side.
